what are the causes of this wealth concentration?

Is it Federal tax system? 
Is it capital tax being much lower than income tax?
Is it money earned abroad being not brought home to US for tax purposes?

Is it because wages are depressed in USA?  
Are the wages depressed by globalization? 
IS it because more money is made through money invested outside USA?
Is it because the US labor is not getting a cut from US world wide business interests?

What are your prescriptions for reducing the income inequality?
