Apparently in about 75 days, the Manglaayan is going to reach Mars. There has been a string of failures in Mars orbital injection in the past.

Can any of the knowledgeable folks summarize the challenges / issues involved OR provide links  that explain the same ? 

I can think of four issues

a) For manual control of this delicate procedure, I presume the latency of commands to reach Mars will be a bottleneck.

b) Accuracy of the actual Manglaayan approach orbit would  be an issue. I presume the error has to be less than 0.25% degrees for both azimuth and elevation of the orbit vis-a-vis the expected orbit.

c) Velocity of the Manglaayan vis-a-vis expected velocity and availability of fuel to control speed.

d) Unknown issues, equipment malfunction OR collision with space rocks etc.. are always there
