They should either eliminate or reduce reservations in medical education (and other professional education as well except may be law) and only merit should be given more weight. When your life is on the line it's best to go with the best.

NONE of this is going to happen.

The best go to a handful of hospitals and the rich and the well to do go there. I went through some of the best hospitals in Coimbatore and looked at their rosters. These are hospitals that do even liver and heart transplants. I saw only a hand of real MDs and super specialists. The MDs are from non-AIIMS or 3rd rate colleges and most are MBBS + PGDXY. The outcome of serious illnesses is the key issue here. recent goofups within our family: ortho surgery messed up and might require another surgery. Another one the vein grafting was messed up and needed to be redone. These are top hospitals in Madras and CBE.

Now that is where the problem is...not the routine ear ache, sore throat stuff. I noticed a couple of relatives diabetes management and dosages - and to say the least was arbitrary. These things will get worse - as the population increases and ages and the real specialist stay up in the top 25 big cities.
